    The bare minimum labs needed for comprehensive female hormone therapy include: FSH, LH, Estradiol, free + total Testosterone, SHBG, DHEA, TSH, free T3, TPO antibodies.

    The bare minimum labs needed for comprehensive male hormone therapy include: CBC, free + total Testosterone, SHBG, DHEA, TSH, free T3, TPO antibodies, PSA w/ 2.5 reflex.

    In addition to the above labs, for comprehensive HEALTH/METABOLIC evaluation, we suggest: CMP, Vit B12/Folate, Vitamin D3, HbA1C, fasting Insulin, Cortisol, Lipid Panel, Apolipoprotein B, uric acid, and CRP.

    If you are desiring to prepay for lab work through LabCorp and take advantage of the incredibly discounted cash pricing we have with them (70-80% off retail), you can do so by PREPURCHASING a lab order - and we will send you a PDF of your lab requisition, to print out, fill out name and date of birth, and take to any LabCorp location to draw. This can be accomplished by CONTACTING US to arrange for prepayment.

    When you finish getting labs drawn and plan to return for consultation and treatment, please be sure to schedule an appointment at least 7 days afterwards (see below). Prepaid labs with LabCorp will automatically return to Papillon for review; if you are getting labs done by your physician/clinician or directly through your insurance, a copy of the complete results must arrive 24 hours prior to your appointment in order to prepare for your individualized dose and plan.     If you would like to discuss your health in detail without the formality of labs or committing to therapy, and would like a formal assessment with Dr. Ho, please schedule a Medical Consultation Appointment, available in 20, 30, and 40 minute sessions. Please note, this Medical Consultation will NOT include a Treatment Consultation nor Pellet Insertion, unless you schedule it that way.

  • Appointments are currently available for our first independent location at Regency Specialties in Surprise, Arizona.

    At the top right of this website, click “Schedule Appointment.” Bioidentical hormone appointments are prepaid (100% refundable) in order to maintain schedule integrity. Since there is so much preparation required to set up for the day’s schedule, and because a missed appointment takes away from another patient, we charge a $50 fee for late cancellations or missed appointments with less than 24 hours notice.

    If you are a brand new patient presenting for your initial full treatment consultation and very first bioidentical hormone treatment appointment, please select both “Initial Bioidentical Hormone Treatment Consult” and “Pellet Insertion.” If you are coming back for your subsequent treatments (women - every 3-4 months; men - every 4-6 months), please select only “Pellet Insertion.”

    For Male visits, Medical Consultations, and all major procedures, scheduling must occur on Thursdays only. For these types of appointments, we will reach out and adjust any scheduling that falls outside of these guidelines.

  • We accept credit and debit cards, and HSA/FSA accounts. We do not accept cash or check. As this is a health and wellness practice, we are not affiliated with health insurance companies, and therefore unfortunately are unable to participate in any health insurance plans.

  • Please send any and all requests for medications, vitamins, supplements, etc. to regency.surprise@papillonwellnessandlongevity.com with “Prescription Requests” in the subject field. In each request, please include your prescription, strength, your mailing address, as well as the last 4 digits of the payment method you wish for us to use. From there, we will arrange for orders and pickup plans. Please allow for a minimum of 1 week, and preferably up to 2 weeks, for all refill requests - in case of delays.

    Nutraceuticals, supplements, and vitamins are usually picked up or purchased on site; we encourage bulk order with a 10% discount at each visit, to help make sure that you have enough between each pellet treatment.

  • This can be accomplished by accessing your Square payment account, which you first created when becoming a client. Login with your phone number and other credentials, and find your way to your securely held PAYMENT CARD ON FILE, which you can replace/update at any time.

    In addition, on every single receipt you receive after services/orders from Papillon Wellness & Longevity, there is a link at the bottom of the receipt allowing you to update your payment information.
